Hospital Admission

What to Bring
Please bring this information in when you arrive:

• Your Postcode and Contact Phone Numbers.
• Name and Address of your child's GP
  
• Name and Address of your Health Visitor ( if known).
  
• Benefit Details ( if appropriate ).

Also your Child's details regarding:

• Vaccinations.
  
• Previous infectious diseases, operations, hospital admissions or illnesses.
  
• Special dietary needs.

Please bring your Personal Child Health Record (red book) with you so that relevant details can be recorded for yourself and other health workers.

• Please note smoking is not allowed in the hospital for health reasons.
  
• Alcohol is not allowed anywhere on the hospital grounds.
• Please bring with you your Leeds Hospital Information Folder ( if you have one – if not ask one the nurses for one when you arrive).
• Also bring with you all your child’s medication.
